Where are we?
On Friday 25th August 2006 we moved to our new premises on Sultan Road having spent two years in temporary accomodation. This new building is situated a little further up from where our temporary accomodation was.

This website has been designed to provide a practical guide to our practice and its services, as well as giving us the opportunity to keep our current and prospective patients informed with any new developments.
The practice is large and modern, with suitable access for disabled patients and aims to retain the traditional caring values that have always been associated with general practice.
Practice History
This practice has been in existence in Lordswood since October 1976 and was founded by Doctor John Fahey and Doctor Joan Montague and Doctor Kamal Sethi in what is now the dental practice in Goad Avenue, Lordswood. Dr Sethi left the partnership in 1978. Dr Fahey and Dr Montague retired in 1989 and Doctors O.S. Singh and S.P. Singh took over the practice in 1990.
This multiprofessional health centre houses all the professionals of Primary Health Care Team, i.e. health visitors, district nurses, midwifes, counsellor, chiropodist, dental clinic and phlebotomist.
Lordswood Health Centre is part of the new LIFT project and has become Lordswood Community Healthy Living Centre. This modernisation will provide more enhanced services for our patients.

Please could all COPD patients bring your inhaler when you have an appointment with the Doctor.

The practice will be open from 8:30 am to 6.00pm Monday to Friday.
Monday to Friday: 8.30am – 12.30am
Surgery is closed (6pm to 8.30am Mon to Fri and 6pm on Fri until 8.30am on Monday)
Extended Hours: Monday 6.30pm - 8.00pm
Wednesday 7.00am - 8.00am
Since 1981 the Government made it compulsory for all doctors to undergo an apprenticeship comprising a three-year post graduate training including one year G.P. experience in a training practice. We are an accredited training practice; all our registrars are qualified doctors who have undergone five years medical school training passing all necessary exams. They have already worked as house officer for twelve months in hospital, they are fully able to deal with all your problems.
Training Practices in Kent, Surrey and Sussex
This practice has been accredited to train doctors intending to become general practictioners.
Further information about training in this practice is available from the receptionist.
Choose and Book
If your GP agrees that you need to see a specialist, the new 'Choose and Book' system allows you to choose a hospital or clinic and book an appointment with a specialist. You can choose from atleast four hospitals. You can also choose the date and time for your appointment and obtain information about your list of hospitals or clinics. You can do this by telephoning the appointments line on:

Chaperone
Should you require a chaperone to be present during your consultation / examination, please let either the receptionists, nurse or doctor know and this will be arranged for you.
What Areas Does the Practice Cover?
The Practice Boundary encompasses ME5 to include parts of Lordswood, Walderslade and Princes Park. If you have any questions regarding your eligibility to join the practice, please ask at reception.

New Patient Registration Form
To begin the process of joining the practice you need to fill in the above form and hand it into reception.
We have had patients email and fax the form to the Practice Manager. These will be destroyed and not valid as a registration form.
Repeat Prescription Request Form
If you have lost the repeat slip attached to the original prescription you can fill in this form and either POST it to the surgery or place it in the BLACK BOX on the reception desk.
Please do not fax, email or phone the reception for repeat prescription as they will not be processed until given in writing.
Change of Address Form
If you have recently moved house and need to inform of us of your new address, please fill in this form and either POST it to the surgery of HAND it in to the reception staff.
